〜ましょうか is used when the speaker offers to do something for the listener: 'Shall I ...?'. The speaker sees that the listener needs help and asks before acting. To accept, say ええ、お願いします or すみません、お願いします. To refuse politely, say いいえ、大丈夫です or いいえ、けっこうです. The same form can also be a soft 'Shall we ...?' when both people do the action together; see the comparison with 〜ましょう below.
§1Structure
Verb ます-stem + ましょうか (持ちます → 持ちましょうか, 開けます → 開けましょうか, します → しましょうか, 来ます → 来ましょうか). The speaker is the one who does the action, so わたしが is often added: わたしが持ちましょうか.

§3Common mistakes
- Answering an offer with 〜ましょう. If someone says 荷物を持ちましょうか, the answer はい、持ちましょう is wrong, because it sounds like 'Yes, let's carry it'. The other person does the action for you, so accept with ええ、お願いします or ありがとうございます, and refuse with いいえ、大丈夫です.
- Using 〜ませんか to offer help. 荷物を持ちませんか asks the listener whether they would like to carry the bags, so it does not work as an offer. When you are the one who will do it, say 荷物を持ちましょうか.
- Attaching ましょうか to the wrong form. 持つましょうか (dictionary form) and 持ちますましょうか (keeping ます) are wrong. Take ます off the ます-form and add ましょうか: 持ちます → 持ちましょうか, 閉めます → 閉めましょうか.
§4How it's different from similar grammar
- 〜ましょう (let's ...)
- 〜ましょう means 'Let's do it together', and the speaker and the listener both act: 一緒に昼ごはんを食べましょう (Let's eat lunch together). In an offer with 〜ましょうか, only the speaker acts, for the listener: 昼ごはんを作りましょうか (Shall I make lunch for you?). 〜ましょうか can also mean 'Shall we ...?' when both people act, as in 何を食べましょうか (What shall we eat?) or そろそろ帰りましょうか (Shall we go home soon?). Look at who does the action to tell the two meanings apart.
- 〜ませんか (won't you ...? / invitation)
- 〜ませんか invites the listener to do something, often together with the speaker: お茶を飲みませんか (Won't you have some tea?). 〜ましょうか as an offer is about the speaker's own action: お茶を入れましょうか (Shall I make some tea?). The replies are different too. To an invitation you say ええ、いいですね or ええ、飲みましょう. To an offer you say ええ、お願いします.
§5How this shows up on the JLPT
In the N5 grammar section, 〜ましょうか appears in short dialogues. The reply is the key: if the other person answers お願いします or ありがとうございます, the blank is an offer with 〜ましょうか, not 〜ませんか or 〜ましたか. In listening, the short-utterance questions show a picture of someone who needs help (heavy bags, a hot room) and ask what the person with the arrow says; the answer is an offer such as 持ちましょうか. In the quick-response questions you hear an offer and must choose a natural reply, such as すみません、お願いします or いいえ、大丈夫です.
