なさい is a command from a person in a higher position to someone they guide: a parent to a child, or a teacher to a student. It is softer than the imperative form (食べろ), but it is still a command, not a polite request. It is also the standard form in test instructions, such as 答えなさい and 選びなさい. It is not used to superiors, customers, or strangers.
§1Structure
Verb ます-stem + なさい (食べます → 食べなさい, 書きます → 書きなさい, します → しなさい, 来ます → 来なさい [きなさい]). なさい has no negative form of its own. To tell someone not to do something, use another pattern such as 〜てはいけません or 〜ないで(ください).

早く起きなさい。もう8時ですよ。

§3Common mistakes
- Attaching なさい to the dictionary form or the た-form. ✕ 食べるなさい ✕ 書くなさい ✕ 食べたなさい → ○ 食べなさい ○ 書きなさい. Take ます off the ます-form and add なさい.
- Thinking that なさい is polite and using it to a teacher, a boss, or a customer. It is a command from above, so it sounds rude in that direction. ✕ 先生、もう一度説明しなさい → ○ 先生、もう一度説明してください / 説明していただけませんか.
- Trying to make a negative command with なさい. ✕ 行かないなさい ✕ 行きなさくない → ○ 行ってはいけません ○ 行かないで(ください). なさい is only used to tell someone to do something.
§4How it's different from similar grammar
- Imperative form (食べろ)
- Both are commands. The imperative form is rough and strong, and it is used mainly by men or in emergencies: 早くしろ (Hurry up!). なさい is softer and sounds like guidance, so parents and teachers of any gender use it to children: 早くしなさい (Hurry up — a parent to a child). The forms are also different: 食べろ / 食べなさい, 書け / 書きなさい, しろ / しなさい.
- 〜てください (polite request)
- てください asks the listener politely, and it can be used to people above you: ここに名前を書いてください (Please write your name here). なさい does not ask; it tells. It is used only downward: ここに名前を書きなさい (Write your name here — a teacher to pupils, or a test instruction).
§5How this shows up on the JLPT
You will see なさい in the instructions of many Japanese tests and textbooks: 正しいものを選びなさい, 質問に答えなさい. In grammar-form questions, the point is the form before なさい: choose the ます-stem (読みなさい), not the dictionary form or the ない-form. Questions may also ask who is speaking to whom: なさい fits a parent to a child or a teacher to a student, and it does not fit a student speaking to a teacher. In listening, the short casual form 〜な (早く食べな = 早く食べなさい) can appear; do not confuse it with the prohibition 食べるな.
