〜ないで joins two actions of the same person and has two uses. (1) 'Without doing A, do B': B is done in a state where A was not done (朝ごはんを食べないで、学校へ行きました). (2) 'Instead of doing A, do B': the person chooses B and does not do A (バスに乗らないで、歩きました). The polite request 〜ないでください ('please don't') comes from the same form and has its own page.
§1Structure
Verb ない-form + で: 食べない → 食べないで, 行かない → 行かないで, しない → しないで, 来ない(こない) → 来ないで(こないで). It is used only with verbs. い-adjectives, な-adjectives, and nouns do not take ないで (they use 〜なくて).

朝ごはんを食べないで、学校へ行きました。

§3Common mistakes
- Making the form from the dictionary form, or writing て instead of で. Wrong: 行くないで / 食べないて. Right: 行かないで / 食べないで. First make the ない-form, then add で.
- Using ないで with adjectives or nouns. They use なくて. Wrong: この部屋は広くないで、不便です / 兄は学生じゃないで、会社員です. Right: この部屋は広くなくて、不便です / 兄は学生じゃなくて、会社員です.
- Using ないで to give the reason for a feeling. When the first part is the cause of a feeling such as 困る, 残念, or さびしい, なくて is normally used: 友だちが来なくて、さびしかったです (not 友だちが来ないで、さびしかったです).
§4How it's different from similar grammar
- 〜なくて (not ... and so / because not ...)
- 〜ないで tells how the second action is done: without the first action, or in place of it. 宿題をしないで、学校へ行きました (I went to school without doing my homework). 〜なくて gives a cause, and what follows is usually a feeling or a state that the speaker does not control: 宿題が終わらなくて、困りました (My homework wasn't finished, so I was in trouble). なくて is also the form for adjectives and nouns (高くなくて, 学生じゃなくて).
- 〜ずに (without doing — written, formal)
- 〜ずに has the same meaning as 〜ないで ('without doing'), but it sounds more formal and is common in writing: 何も言わないで帰りました (spoken) / 何も言わずに帰りました (written). The form of する is different: しないで but せずに. Only ないで is used in requests: 行かないでください (you cannot say 行かずにください).
§5How this shows up on the JLPT
The most common N4 question asks you to choose between 〜ないで and 〜なくて. Look at the second half of the sentence: if it is an action that the same person does (went, ate, slept), choose ないで; if it is a feeling or a result such as 困りました or 残念です, choose なくて. Also check the word before the blank: adjectives and nouns never take ないで. In reading passages and in the text-grammar section, the same meaning may appear as 〜ずに, so read 食べずに as 食べないで.
