〜場合は takes one possible situation out of several and tells what to do, or what happens, in that situation. It sounds a little formal and is common in rules, notices, and instructions, often for trouble or emergencies (fire, illness, delay, a lost card). It is not used for things that are sure to happen or for a single real event that already happened. With a verb, the た-form is used when the situation must already have happened at that moment (なくした場合は), and the dictionary form when it is still going to happen (遅れる場合は).

§1Structure

Verb dictionary form / た-form / ない-form + 場合は (遅れる場合は, なくした場合は, 下がらない場合は). い-adjective + 場合は (小さい場合は). な-adjective + な + 場合は (必要な場合は). Noun + の + 場合は (雨の場合は). は is sometimes left out or replaced (〜場合、 / 〜場合には / 〜場合も).

§2Examples

  • 雨の場合は、試合はありません。

    あめのばあいは、しあいはありません。

    In case of rain, there will be no match.

  • 会議に遅れる場合は、電話で連絡してください。

    かいぎにおくれるばあいは、でんわでれんらくしてください。

    If you are going to be late for the meeting, please let us know by phone.

  • 熱が下がらない場合は、もう一度病院へ来てください。

    ねつがさがらないばあいは、もういちどびょういんへきてください。

    If your fever doesn't go down, please come to the hospital again.

  • カードをなくした場合は、どうすればいいですか。

§3Common mistakes

  • Forgetting の after a noun or な after a な-adjective, or adding だ. Wrong: 雨場合は / 必要場合は / 必要だ場合は. Right: 雨の場合は / 必要な場合は.
  • Using 場合 for a real event in the past, with the meaning 'when'. 場合 is about a possible case, so a real past event takes とき. Wrong: 昨日、家に帰った場合は、母が料理をしていました. Right: 昨日、家に帰ったとき、母が料理をしていました.
  • Using 場合 for something that is sure to happen. There is no 'other case', so 場合 does not fit; use たら (or と, when the second half is a natural result and not an invitation or a request). Wrong: 春になった場合は、花見をしましょう. Right: 春になったら、花見をしましょう.

§4How it's different from similar grammar

〜とき (when — the time of a real or expected event)
〜とき simply gives the time of an event, real or not, past or future: 子どものとき、よく川で遊びました (When I was a child, I often played in the river). 〜場合は picks one possible situation and says what to do in it; it is not used for real past events: 地震の場合は、エレベーターを使わないでください (In case of an earthquake, do not use the elevator). For a possible future situation both can be used, but 場合 sounds more formal and fits rules and notices.
〜たら (if / when — everyday conditional)
〜たら is the everyday form and covers both 'if' and 'when': 道が分からなかったら、電話して (If you don't know the way, call me) / 家に着いたら、電話して (Call me when you get home). 〜場合は is used only for 'if this case happens', sounds more formal, and is typical of written rules: 道が分からない場合は、受付で聞いてください (If you do not know the way, please ask at the reception desk). It cannot replace たら in the 'when you get home' sentence.

§5How this shows up on the JLPT

In grammar-form questions, the connection is the usual point: noun + の, な-adjective + な, and a verb in a plain form (雨の場合は, 必要な場合は, 遅れる場合は). When the choices are 場合, とき, あいだ, and まえ, choose 場合 if the sentence is a rule or an instruction about a possible problem and ends with 〜てください or 〜ことができます. In reading, 場合 is very common in information-search questions (notices, rules for a library or a gym, how to cancel); find the line with the 場合 that matches the person's situation, and the answer is usually right after it.
