〜ようにする means that you make an effort so that you do, or do not do, something. It does not say that you succeed every time; it says that you try and pay attention. 〜ようにしています is used for an effort that continues as a habit. 〜ようにしてください asks the listener to take care about something from now on or regularly, and it is softer and less direct than 〜てください.
§1Structure
Verb dictionary form + ようにする (食べるようにする). Verb ない-form + ようにする (食べないようにする). Common forms: 〜ようにしています (a habit you keep up), 〜ようにします (a promise or decision from now on), 〜ようにしました (you started making the effort), 〜ようにしてください (a soft request to take care to do / not to do something).

毎日、野菜を食べるようにしています。
まいにち、やさいをたべるようにしています。

§3Common mistakes
- Using 〜ようにしてください for a simple request that the listener should do right now. ✕ すみません、そのペンを貸すようにしてください → ○ すみません、そのペンを貸してください. 〜ようにしてください is normally used for things to be careful about regularly or from now on: あしたは8時までに来るようにしてください.
- Mixing up ようにする and ようになる. ✕ 練習して、自転車に乗れるようにしました (when you mean 'I became able to ride') → ○ 自転車に乗れるようになりました. する is about your effort; なる is about a change that happened.
- Using the wrong form before ように. ✕ 食べてようにしています ✕ 食べないのようにしています ✕ 食べますようにしています → ○ 食べるようにしています ○ 食べないようにしています. Only the dictionary form or the ない-form comes before ようにする.
§4How it's different from similar grammar
- 〜ようになる (come to / become able to)
- 〜ようにする is an effort made on purpose by the person. 〜ようになる is a change that happens over time, and it often follows a potential verb. 毎朝6時に起きるようにしています = I try to get up at 6 every morning (my effort). 毎朝6時に起きられるようになりました = I have become able to get up at 6 every morning (the result).
- 〜ことにする (decide to)
- 〜ことにする states a clear decision: you decided to do it. 〜ようにする is weaker: you will try, as far as you can. お酒をやめることにしました = I decided to stop drinking alcohol (a firm decision). お酒を飲まないようにしています = I try not to drink alcohol (I make an effort, but there may be exceptions).
§5How this shows up on the JLPT
Grammar-form questions often ask you to choose among ようにする, ようになる, ことにする and ことになる. Check whether the sentence is about the speaker's own effort (ようにする), a change or new ability (ようになる, often after a potential verb), or a decision (ことにする). Another type tests the form before ように: only the dictionary form or the ない-form fits. In listening, a doctor or teacher often gives instructions with 〜ようにしてください, and the speaker's healthy habits appear as 〜ようにしています. In sentence ordering, keep verb + ように + して + います together.
