〜ていただけませんか

Verb て-form + いただけませんか (教えていただけませんか). Less polite forms from もらう: 〜てもらえませんか, 〜てもらえますか; casual: 〜てもらえない？ Politeness, from low to high: てもらえますか → てもらえませんか → ていただけますか → ていただけませんか. To ask someone not to do something: verb ない-form + でいただけませんか (吸わないでいただけませんか).

  Excuse me, could you please take a picture for us?

• もう少しゆっくり話していただけませんか。

  もうすこしゆっくりはなしていただけませんか。

  Could you please speak a little more slowly?

• この書類の書き方を教えていただけませんか。

  このしょるいのかきかたをおしえていただけませんか。

  Could you please show me how to fill in this form?

• 作文を書いたんですが、ちょっと見ていただけませんか。

  さくぶんをかいたんですが、ちょっとみていただけませんか。

  I wrote a composition. Could you please have a look at it?

• 田中さん、ちょっと手伝ってもらえませんか。

  たなかさん、ちょっとてつだってもらえませんか。

  Mr. Tanaka, could you help me for a moment?

• すみません、塩を取ってもらえますか。

  すみません、しおをとってもらえますか。

  Excuse me, can you pass me the salt?

• ごめん、辞書を貸してもらえない？

  ごめん、じしょをかしてもらえない？

  Sorry, can you lend me your dictionary?

• すみませんが、ここでたばこを吸わないでいただけませんか。

  すみませんが、ここでたばこをすわないでいただけませんか。

  Excuse me, but could you please not smoke here?

• Forgetting the potential form: ✗ 教えていただきませんか. A request asks whether you 'can receive' the favor, so いただく must become いただける: ○ 教えていただけませんか. In the same way, ✗ 教えてもらいませんか → ○ 教えてもらえませんか.
• Using it for your own action. The person who does the て-form action is the listener. 窓を開けていただけませんか cannot mean 'May I open the window?'; it means 'Could you open the window?'. To ask permission for your own action, say 窓を開けてもいいですか.
• Using the ます-stem or dictionary form instead of the て-form: ✗ 書きいただけませんか, ✗ 書くいただけませんか → ○ 書いていただけませんか.

〜てください (please do)

〜てください tells the listener what to do. It is polite, but it does not ask whether the listener is willing, so it suits instructions: ここに名前を書いてください (Please write your name here). 〜ていただけませんか asks if the favor is possible and leaves room to say no, so it is better when you ask a personal favor: 推薦状を書いていただけませんか (Could you please write a letter of recommendation for me?).

〜てくださいませんか (would you please...?)

Both are very polite requests and you can use either in most situations: 教えていただけませんか／教えてくださいませんか. いただけませんか comes from もらう (I receive), so it is a potential form: いただけ. くださいませんか comes from くださる (you give), and it is not a potential form. Many people feel いただけませんか is a little more humble, and it is the more common one today.

N4 grammar-form questions often mix いただけませんか with wrong look-alikes such as いただきませんか or くださりませんか; remember 'potential form + ませんか'. Sentence-ordering questions often use the set 〜んですが、〜ていただけませんか. In listening (task and response questions), you must know who does the action: with 〜ていただけませんか, the listener does it for the speaker.
